If you intend to use your forms to design your own on-line surveys, please do not use form_builder or vxml. The design choices made by these third party toolkits may not be what you want. This is especially true when you download them from the Web. You may not be installing them on a shared drive or machine that you wish to share with others.
This is a demo of Oracle Forms 10g with Views based Reports. These reports use 0-based numbering as this is more preferable to the 1-based numbering used in many reports. Remember the design you create using Oracle Forms will not use this numbering scheme. Oracle Reports on the other hand will always use a 1-based numbering.
